Adding a new form to a chart
- Click new.
- Select form of choice. Click here to learn about form management.
- Click the preview to view the form prior to applying it to the patient chart if not certain of the form name.
- Highlight the form and click ok, or double click the form name.
- Fill in the form as necessary.

- Click save.
- Click print to hand to patient.
- Or go to patient messaging to send to the patient electronically. Click here to learn more about patient messaging.
