  1. Book an appointment online specifically for a flu shot.

    2. Confirm identity.

    2. Select a date followed by a time.

  1. Patient received appointment reminder (via text/email/voice)
    1. Open the text or email. (Below is the email example).

    1. Note there are two actions here, first, click to confirm or cancel.

    1. Confirmation of appointment has been received.
  1. Complete the electronic influenza vaccine consent form.
    1. Click the fill out intake form.
    2. It will ask to confirm identity.

    1. Click continue to start the intake form for the influenza vaccine consent.

    1. Complete the intake form questions, reading the information and signing at the bottom.

  1. Check in electronically.
    1. Patient can scan the QR code posted on the walls, use a tablet provided by the clinic to check in, or use the link sent to them in the appointment reminder to check in.
    2. Follow the steps to confirm identity and demographic information.

    1. Patient can sit in the waiting room until staff call the patient into the exam room.
