Automated flu vaccine consent - The patient's perspective
- Book an appointment online specifically for a
flu shot.
- Click the online booking button from the clinic’s
website.
- Confirm identity.
- Select the practitioner type, practitioner,
and appointment type.
- Select a date followed by a time.
- Confirm the details of the appointment and the
email to send the confirmation to.
- A confirmation the appointment has been booked
and email has been sent.
- Patient received appointment reminder (via
text/email/voice)
- Open the text or email. (Below is the email
example).
- Note there are two actions here, first, click
to confirm or cancel.
- Confirmation of appointment has been received.
- Complete the electronic influenza vaccine
consent form.
- Click the fill out intake form.
- It will ask to confirm identity.
- Click continue to start the intake form for
the influenza vaccine consent.
- Complete the intake form questions, reading
the information and signing at the bottom.
- Check in electronically.
- Patient can scan the QR code posted on the
walls, use a tablet provided by the clinic to check in, or use the link sent to
them in the appointment reminder to check in.
- Follow the steps to confirm identity and demographic
information.
- Patient can sit in the waiting room until staff
call the patient into the exam room.
