Chart Templates for Patient Summary
To support clinician's with Patient Summary submissions, Healthquest has created 3 charting templates for patient history records. These templates are automatically uploaded when your clinic is updated to version 4.4.70. They are categorized as medical history items, which results in them being stored in the medical history section of the charting overview.
PS- Family History
The family history template is for recording chronic health conditions that affect family members of the patient. Use the notes box at the top of the chart note to record general information regarding the patient's family. Use the add button (show in red) to add items for specific family members, use one line per health condition.
PS - Procedure History
The procedure history template is for recording surgeries or procedures that the patient has been offered or completed. Use the notes box at the top of the chart note to record general information regarding the surgical history. Use the add button (show in red) to add items, use one line per procedure/surgery.
PS - Social History
The social history template is for recording lifestyle or social history items (tobacco use, alcohol use, exercise, illicit drug use, etc.) that may impact the patient's health.
