Daily use of referral management

Open Referral Management; select each referral status on left hand side, one at a time.

Pending status - open the referral, add in specialist name, fax to specialist, and type in notes the action completed. Change Referral Status to “Faxed” and change the due date two weeks ahead.
   ·   Goal-By the end of the day this should be empty.

Faxed status - open referral once the due date turns red, check for a status update; if yes, update status to “waiting for appointment” and postdate to appointment time expectancy, and make notes in the referral notes box accordingly. If not, call the clinic to check the status of referral.

        ·   Goal-No Red referrals by the end of the day.

Waiting for Appt status - open referral when the due date turns red, see if appointment info is available, if yes change Referral status to “Notify Pt” and change due date to today. If there is no appointment notice, check in with specialty clinic and update accordingly. Make notes in the referral notes box accordingly.

        ·   Goal-Make sure all referrals either have an appt or have been followed up with at the specialty clinic and postdated two more weeks.

Notify Pt - open referral once the due date turns red, contact patient to inform them of appointment details, once patient is aware, make a note in referral notes, change referral status to “Waiting for Consult Report” and postdate 30 days after the appointment date. Make notes in the referral notes box accordingly.

        ·   Goal-All patients contacted, or attempted to contact, by the end of the day.

Waiting for Consult Report - open referral 30 days after consult date, check for consult letter back from specialist, if yes mark referral status as completed to reconcile the referral. If not, call the specialist clinic to ask for one, check again in another 2 weeks. Make notes in the referral notes box accordingly.

        ·   Goal-All patients have a consult report on file and are completed or postdated two weeks after communicating with the specialist clinic.
