Due for Colorectal Cancer Screening
Colorectal Cancer Screening – (Due for FIT test (Lab test), Sigmoidoscopy, or Colonoscopy (Procedure reports))
Below is an example of the complete query
example for those due for colorectal cancer screening.
*Note, we are looking for patients between 50
and 74 years, with no lab or procedure results found on either the labs tab,
the ASaP template, or the scanned documents. This may differ based on clinic
workflows.
- Click ‘New’ on the queries window,
enter a name for the query in the bottom description box, and while creating
the query, uncheck the notify on chart and appointments.
- Click edit query, then click ‘new
line’, select client and ok. Enter the demographic information, as below.
- If you want to specify a doctor,
select one from the dropdown as well. Click save line.
- Next, create new lines to look for
patients without FIT, sigmoidoscopy, or colonoscopies.
*Note the criteria shown below on the query
line is the information found on the labs tab of a patient chart.
- Enter the result date criteria, in the
last 2 years, and check off the NOT in the top right.
- The test description should match the
description found on the labs tab of a patient’s chart.
- Click save line.
- Next, create new lines to look for
patients without the FIT or colonoscopy documented in the ASaP
template.
- Click new line, select chart notes.
- On the left, select the ASaP template.
- At the bottom, for the other dates,
select the FIT date, then complete the phrase, in the last 2 years, and check
off NOT in the top right corner.
- Click save line.
- Repeat this for the colonoscopy date
in the ASaP template, in the last 10 yrs.
- Next, create new lines to look for
patients without the sigmoidoscopy or colonoscopy reports in the scanned
documents.
- Click new line, select scans.
- Enter the result date criteria, in the
last 5 years, and check off the NOT in the top right.
- Below, looking for the notes section,
this is the description seen on the charting overview.
- Or
select a scan type for sigmoidoscopy if you have one.
- On the image date enter ‘in the last 5
years’ and check off NOT in the top right corner.
- Click save line.
- Repeat the above, this time with the
scan type colonoscopy, and the image date in the last 10 years, checking off
the NOT in the top right corner.
- Click save line.
*Note
Scan types may differ per clinic setup. This example shows the use of scan
types (dropdown field) as well as the description (free text field).
- Save line and close.
- Run the report and double click on a name from the report and click
charting to check a few charts to make sure it is running accurately.
- If correct, check off notify on chart and notify on appointment as
applicable.
