Form templates

Form templates can increase efficiency when sending patients for common screenings.

For example:

Many patients with diabetes will have a standard set of lab tests they go for on a routine basis, having all those tests pre-marked on a form template allows you avoid duplicating work.

 

Applying a form template to a patient’s chart

  1. Open the chart as normal, then from the forms tab click new, or on the reference panel click the plus sign on the form section.
  2. The form selection window will open as normal.
  3. Select your template from the right-hand side at the top.
  1. If a doctor has been applied to the form template, it will appear at the top.
    1. If you select the option to display templates exclusively for one doctor, only templates featuring either an empty doctor field or the doctor chosen from the dropdown will be shown.

 

Building a form template

On the tabletop screen:

  1. Click setup > forms > setup form templates.
  2. Click ‘New’ and select the blank form you wish to use.
    1. Use the search box along the bottom if required.
  3. Click ok, returning to the setup form template window.
  1. Change the name of the form at the top. (This example is ‘Diabetes lab req’).
  2. Select a specific doctor in the dropdown to bring it to the top of the list of templates while applying a form to a patient’s chart.
  3. Check off any tests that are commonly on this type of requisition.

*Note, the patient and authorizing provider information will automatically populate into the form once applied to a patient’s chart.

Use the “Form Fields” button to set up the template to automatically fill in other information from the patient chart. For example, perhaps the referral doctor information needs to populate in the copy to fields.
  1. Place the cursor in the first field to populate (the copy to name field).
  2. Click on the form fields button at the top of the form.

  1. Select the type of information to populate, in this case select the referral doctor radio button at the top.
  2. Choose which information needs to go in the specific spot on the form, for example the referral doctor name.
  3. Click ok.

  1. Repeat the above steps to continue adding referral doctor information into the different spots on the form.
  2. Click save and close.

 Test this form out on a test patient to make sure everything is populating correctly. See above for steps on how to apply a form template to a patient’s chart.
