To access your intake form management window, go into Admin -> Intake Forms -> Intake Form Management. If you do not see this option, your clinic does not have intake forms enabled.

Within this window you can access your clinic's current intake forms (teal), groups of forms that your clinic has (purple), and our public intake form library (blue).

In this section you are able to manage the intake forms that you have downloaded for use. You can see if there are available updates to the form, or archive them if no longer required.

If you regularly send out multiple forms for certain appointments or conditions you can create groups or bundles of intake forms. Use this section to create new groups or edit/remove existing groups.

Click the new group button 

You can then name the group that you are creating and add a description.

Next, select an available form from the list on the left, use the 'add button' to include it in your form group. Feel free to add the forms to your list in any order.

Once you have added all the required forms to the group, use the two lines to the left of the form title to reorder the forms. Do this by clicking the lines and dragging the form to change the order. The order of the list is the order in which the patient is asked to complete the forms.

If you accidently add an incorrect form to the group, select the form in the list on the right and use the 'remove' button to take it out of your group. If you have a form selected there will be a red line present on the left side.

Save the group once all required changes have been made.

To edit an intake form group, click the edit button beside the group name.

You can then edit the group name or description. You can also add or remove forms from the group. Click 'save group' once you've finished updating the group.
