New forms must be created by Healthquest. To request a new form to be added to the database, email forms@microquest.ca with a pdf document attached or a weblink to the pdf. Time for completion can vary.
Ensure you have checked your Forms Management window for any updates prior to submitting your request.
You will receive an email back once your form has been created.
To add the form to your database once created, go to Setup -> Forms -> Form Management.
Click the 'Load Form Updates' button to retrieve the new forms. Find the desired form in the list and change the usage to often, this will make it easily accessible to all users in the clinic. If there is an old version of the form, search for it in the search bar and change the usage to never. When finished, click 'save & close'.
