Creating motor vehicle accident (MVA) forms

To complete MVA claim forms, specific forms will need to be generated followed by a third-party invoice. Creating invoices for MVA claims is the same as billing any third party. Click here to review third party billing.


  • Create the third-party invoice, click the above link to learn how to create third-party invoices.
  • Generate the MVA claim form.
    • Click the client menu, select ‘motor vehicle accident form’.
    • Search for the correct patient (it will default open the last patient that was open).
    • Click new.
    • Select the applicable form options required for the encounter.
    • Choose if you wish to start over with a blank form, or if you want to a previous form to populate the new form.
    • Click ok.

    • Select the practitioner along the side.
    • Scroll through the form filling out as much information as possible.
    • Click save.

 

If any of the forms require printing, open the MVA forms window, search for the correct patient, highlight the applicable form, click print.

 

Generated MVA forms can also be found under the forms tab on a patient chart.
