Creating RCMP claim forms

Specific forms will need to be generated followed by a third-party invoice. Creating invoices for RCMP claims is the same as billing any third party. Click here to review third party billing.
  1. To generate the RCMP claim form, click the client menu, select ‘RCMP claim form’.
  2. Click new claim.
  3. It may ask if you wish to copy from an existing chart note, select as appropriate.
  4. Fill in all the available information.

    1. Save – saves all the information in the form.
    2. Print invoice – opens the invoice selection window to select the third-party invoice.
      1. Select the correct items from the invoice list.

      1. Click print and select yes or no to print patient copy.
      2. Select yes or no to print all pages.
      3. Select which pages to print (Blue cross/patient copy, provider copy, HCC copy, and Member copy) and click ok.

    1. Print PT copy – opens the print window to print the patient copy of the official form.
