Form Groups

Form Group Introduction

Form groups are bundles of forms that are commonly provided together. An example of this would be a prenatal form group. You would include a blood type requisition, ultrasound, communicable diseases, lab requisition and any other applicable items.

Groups can be formed using form templates, where there is preselected/prefilled information on the forms, or with blank forms.

Creating a Form Group

To create or manage a form group, go into Setup -> Forms -> Setup Form Groups.

To create a new Form Group, select ‘Add Form Group’.


Use the window that pops up to name the form group that you would like to create and set a provider that the group is for. Click okay once complete.

You will return to the previous window to add forms to the group. Select your group from the list of Form Groups on the left. Click ‘Add Form’ to make your selections.

A form selection window will populate, select the desired forms one by one using this window. This window is the same as the one that opens when you want to generate a form for a patient. Blank forms will populate on the left (red) and templates on the right (purple). Use the ‘Only Show Templates For’ checkbox if you would like to filter the form templates to a specific provider.


Select the form you would like and click ok.

You will return to the form group management window. The form selection will now appear in the list on the right side.


Continue to add the necessary forms until all desired selections have been made. If you need to change the order that the forms will populate in the patient chart, use the arrows on the right side to resort.

Your form group is now ready to use.

Using a Form Group

To use a form group to generate forms for a patient, select ‘forms’ in a patient chart or client card, click new, and select the group from the bottom right side.

 

The forms that were added to the group will then populate in the patient chart. They can be edited or added to as needed.
