Adding Framingham Risk Calculator to chart templates

On the Healthquest Tabletop screen click on setup > charting setup > template design.

Click on ‘select template’ and open the template wanting to add the Framingham calculator to.


Click ‘new field’ in the top right corner.

Select the calculated field called Framingham score, click finish.


Click where the field should be displayed, to move it, highlight it and use the arrow keys or drag and drop with the mouse.


Click save and close.

 

If wanting to add the two check boxes, one for smoking, and one for family history, after clicking save click ‘New Field’ again.

Select checkbox, click next, enter Smoker? in the name the field type.

Click finish and place the field in the desired location.


Right click on the checkbox, a window will pop up.



In the window, use the 'Field Type' dropdown to select Smoker. Click ok.

Save and click ‘new field.’

Select checkbox, click next, enter 1st Degree Relative with Premature CVD? in the name the field type.

Finish and place the field in the desired location.


Right click on the checkbox. In the window that opens, use the 'Field Type' dropdown to select CloseRelPrematureCVD. Click ok.


Save and close.
