To support provider workflows, Healthquest has created a charting template to assist with completing short term disability paperwork.
The charting template contains all the necessary fields to complete a short term disability form for 5 of the major insurance providers in the province of Alberta. Complete your documentation and print the form using a custom print design for the specified insurance company.
The insurance providers with short term disability forms supported in this Healthquest feature are:
- AB Blue Cross
- Canada Life
- CLHIA
- Manulife
- Sunlife
How to Utilize the Chart Note Template
To utilize the chart note template, click the ‘new’ button used to create a new chart note. Select Third Party Short Term Disability from your list of blank chart notes. **If this template is not available, please contact Healthquest support.
Complete your documentation within the chart note. Once complete, click the print button and select the appropriate option from the print style window.

The chart note will then print in the format of the specified template.
**Only the information required for the form will be printed. The charting template contains fields that do not pertain to all the listed forms.
Follow Up Appointments
For future short term disability updates, the chart note information can be utilized in a couple different ways.
1. Use the field history to copy only desired information.
To create a follow up visit, create a new chart note and select a blank copy of the Third Party Short Term Disability template.
Ctrl + Left Click on a field that you would like to view the history of, or that you would like to copy the values into your new chart note.
2. Create a copy of the previous note and change the necessary details
When creating a follow up visit, use the bottom right corner of the new chart note window to copy a previous version of the template. The text entered in the previous visit will show in red in the new chart note.
Review the information documented and change the required details. Print the report once completed.
